To prepare for your appointment, please keep the following in mind:
Have your most recent medical provider send over your records prior to your appointment
Remember to bring:
- Health Insurance card(s)
- If uninsured, please be prepared to pay a $60 co-pay at the time of your visit. For information about financial assistance, please call (804) 828-0966.
- Infusion logs, if applicable
- Pertinent medical records
- Names, addresses & phone numbers of your Primary Care Physician and Dentist
- A pair of shorts for physical therapy examination
- Pediatric Patients: Immunization Records
What to expect at your first visit:
- During your comprehensive care visit, you can expect to be evaluated and treated by a team that includes:
- Hematologist
- Nurse
- Social Worker
- Physical Therapist
- Genetic Counselor
- Nutritionist (Pediatric patients only)
- Educational Consultant (Pediatric patients only)
- Please be prepared to be at your comprehensive visit for approximately 4 hours.
