To prepare for your appointment, please keep the following in mind:

  • Have your most recent medical provider send over your records prior to your appointment

  • Print, complete, and bring in the Virginia Bleeding Disorders Program application: 

  • Remember to bring:

    • Health Insurance card(s)

      • If uninsured, please be prepared to pay a $60 co-pay at the time of your visit.  For information about financial assistance, please call (804) 828-0966.

    • Infusion logs, if applicable

    • Pertinent medical records

    • Names, addresses & phone numbers of your Primary Care Physician and Dentist

    • A pair of shorts for physical therapy examination

    • Pediatric Patients: Immunization Records

What to expect at your first visit:

  • During your comprehensive care visit, you can expect to be evaluated and treated by a team that includes:

    • Hematologist

    • Nurse

    • Social Worker

    • Physical Therapist

    • Genetic Counselor

    • Nutritionist (Pediatric patients only)

    • Educational Consultant (Pediatric patients only)

  • Please be prepared to be at your comprehensive visit for approximately 4 hours.
